COCKTAIL REGIMEN IN TREATMENT OF SUDDEN SENSORINEURAL HEARING LOSS.

Abstract

Aim: To evaluate the efficacy of cocktail regimen (intratympanic steroid +oral steroid+ valacyclovir+pentoxifylline) in patients suffering from sudden sensorineural hearing loss. Materials & Methods: 35 patients suffering from sudden sensorineural hearing loss were admitted in this study. All the patients were given a cocktail regimen treatment consisting of- Intratympanic steroid injection for 5 days, which was then tapered off with oral methylprednisolone, T. Pentoxifylline 400mg BD for 5 days T. Valacyclovir 500mg BD for 5 days. Pre-treatment and 1month, 3month and 6month post-treatment Pure Tone Audiogram (PTA) were compared to analyse the efficacy of the above mentioned cocktail regimen in treating sudden sensorineural hearing loss based on Modified Siegel’s criteria. Results: 26 out of the 35 patients showed significant improvement in hearing and improved pure tone audiometry readings after 6 months of treatment. The remaining 9 patients did not show any signs of improvement. Conclusion: This cocktail regimen can be administered in patients suffering from sudden sensorineural hearing loss as it shows beneficial results over long term steroid use and retorting to the use of hearing aids. Clinical Significance: Since idiopathic causes account for the majority of cases of sudden sensorineural hearing loss, pursuing a cause-specific diagnosis would be futile. Rather, by following this protocol, we can prevent the need for long-term systemic steroid treatment, limit the disease's progression, and preserve valuable time.
